Proposed outputs

  • Theory of change illustrating primary care-care home relationships that enables access to healthcare to meet residents’ needs and priorities and builds and tests evidence of what works at the system level of care. We will identify key audiences for this and plan how to reach them. A visual representation of the theory will be shared widely, such as through the project website, health providers, commissioners, and networks.
  • Project report and publications targeting researchers, healthcare providers, and stakeholders. A paper describing the findings submitted to a scientific, peer-reviewed journal for publication. Summaries sent to care home relevant magazines.
  • Presentations will be given about findings at professional conferences.
  • Website providing transparency and allowing communication with interested parties and to explain the project’s key aims and methods and update on progress.
